The Prognostic Scoring of Polytraumatized Patients and the Validity of ISS and TRISS for Prediction of Survival

Summary:

       Polytraumatized patients are endangered by development of the multiorgan dysfunction syndrome (MODS) immediately after the accident because of the traumatic shock and later especially by progression of the septic complications. Predisposition for septic complications is caused by neurohumoral mechanisms – reaction on polytrauma and potentiated by the invasive therapeutic approaches during the resuscitation care. Topic of the project was to document all the predisponable influences for individual organ and systemic dysfunctions, optimalization of the therapeutic approaches for the prevention and therapy of the individual organ dysfunctions in the large group of patients. 1113 of traumatized patients admitted on ICU of the Trauma Hospital in Brno were analysed on the prediction of the mortality (survival). TRISS (Trauma and Injury Severity Score) was a good predictor of the trauma mortality. ISS (Injury severity score) showed lower sensitivity as predictor of mortality mainly in the patients with the head trauma.
