Ampullomas – Diagnosis and Endoscopic Therapy

Summary:

       Early and accurate diagnosis of tumours of the ampulla ofVater will lead to early treatment and subsequent better prognosis. ERCP has been shown to be one of the best diagnostic modalities. Endoscopic papillotomy improves diagnostic accuracy of histological examination. Endosonography and intraductal ultrasonography can reveal the size and extent of a tumour, accurately demonstrate invasion into peripancreatic tissue, and give a therapeutic alternative. Endoscopic resection and ablation of benign ampullary tumours appears to be a viable alternative to surgical therapy with acceptable morbidity. The main problem with endoscopic therapy is the exclusion of malignancy in the adenoma. Multiple forceps and snare biopsies after papillotomy reduce the risk of false-negative results.
