Summary:
The case of 8 years old female with an acute bullous eruption on the trunk and limbs, forming
bizarre rosette-like pattern is described. Results of clinical, histological and direct immunofluorescence
examinations were suggestive for the diagnosis of pemphigus. The patient was treated with
the leukotriene antagonist montelukast for chronic asthmatic bronchitis started 5 months before
the onset of skin eruption. The montelukast was withdrawn and the disease gradually subsided after
administration of systemic corticosteroids and dapsone. Sporadic pemphigus is very rare in small
children. To our knowledge, there have been no previous case reports of pemphigus associated with
montelukast administration.
