Frequent Small-volume Drainage in the Treatment of Resistant Ascites

Summary:

       1. Objective. The objective of our work is to present a new therapeutic modality in resistant ascites based on the technique of peritoneal dialysis: frequent small-volume drainage of ascites. 2. Method. In 17 patients with resistant ascites a catheter for peritoneal dialysis was inserted into the peritoneal cavity using a suitable technique. Using bags which are used for peritoneal dialysis with the so-called disconnect system ascitic fluid was drawn in portions of 31 per volume (volume of one bag) and at a rate depending on the rate of ascites formation and the patient’s symptoms. 3. Results. The peritoneal catheter was successfully and without complications inserted in all patients. Drainage of ascites eliminated symptoms of tension ascites. Complications associated with the procedure pertained to the frequency and were treated as in the case of peritoneal dialysis: acute peritonitis in three instances, infection of the outlet of the catheter in one patient. One-year survival of patients with cirrhotic ascites was 69 %, survival of all patients 55 %. 4. Conclusion. Frequent small-volume drainage of ascites is a method for treatment of resistant ascites, which deserves further investigation.
