Summary:
Objective: To assess the ability of B-mode sonography and power Doppler sonography to detect acute
pyelonephritis in children and to compare the findings with those from 99m Tc-DMSA scintigraphy.
Patients and methods: Sixty-three patients (36 girls, 27 boys, age range 6 - 16 years, mean age 10.2 years)
admitted to hospital with a clinical diagnosis of acute pyelonephritis (AP) were assessed using B-mode sonography,
power Doppler sonography and 99m Tc-DMSA scintigraphy, the latter providing the reference method.
Results:DMSA scintigraphywas used as the standard of reference for diagnosis ofAP.TheDMSAscans showed
changes consistent with AP in 95.2% (60/63) of the patients. B-mode sonography showed changes of AP in 34.9%
(22/63) of the whole group and in only 36.6% (22/60) of patients with scintigraphically documented AP. Power
Doppler sonography showed changes in 38% (24/63) of the whole group and in only 40% (24/60) of the patients
with scintigraphically documented AP.
Conclusion: B-mode sonography and power Doppler sonography are relatively insensitive methods for the
detection of acute inflammatory changes of the renal cortex.
