Summary: The authors describe the case of a 45-year-old man with a four-year history of chronic tophaceous
gout. The patient suffered from proteinuria and chronic renal failure, and from renal biopsy AA
amyloidosis was diagnosed. With regard to the finding of paraprotein (IgGk) in serum, a myeloma
was suspected but was not confirmed and therefore it was concluded that benign monoclonal
gammapathy (MGUS) is involved. The occurrence of AA amyloidosis in gout is rare and so far only
a few cases were described. In the mentioned patient it will be however necessary to follow up the
possible development of MGUS and potential relations to AA amyloidosis.
