An Intrahepatic Haematoma as a Complication of TIPS Procedure in a Patient

Summary: The portal venous pressure reduction by creation of the transjugular intrahepatic portosystemic
shunt (TIPS) is a method of the first choice in treatment of patients with
Budd-Chiari syndrome secondary to the hepatic vein thrombosis. Authors report
a case of a 33-year old woman suffering from a chronic hepatic vein thrombosis due to
the anti-phospholipid syndrome, with an intrahepatic inferior vena cava obstruction.
Fifteen days after TIPS creation an intrahepatic haematoma and haemoperitoneum
were diagnosed. Twice repeated hepatic artery angiogram did not show any source of
the intrahepatic haemorrhage and the patient was managed by conservative therapy.
This complication seems to be specific for patients with Budd-Chiari syndrome and it
is important to know about this possibility.
