Percutaneous Fixation of Pelvic Fractures

Summary: Unstable pelvic fractures are usually associated with high energy injuries and influence negatively
the post-injury reaction. As to surgical procedures most frequently methods of inner osteosynthesis
are used which however cannot be applied in the acute stage of treatment, particular in multiple
injuries.
Percutaneous fixation under skiascopic control lacks precision of fracture stabilization in deeper
layers. The mentioned shortcomings are overcome by percutaneous fixation under CT control. It is
minimally invasive and markedly reduces the risk of infectious complications. CT control provides
direct visualization of fragments of the fracture and foramina of the sacral bone and makes it
possible to place quite reliably the screw to rule out or minimalize damage of nerve fibres. It also
reduces the chance of damage of pelvic vessels. The disadvantage is a greater radiation load as
compared with procedures made under skiascopic control. By optimalization of image-taking
parameters it can be however markedly reduced.
In the presented paper the authors submit their own initial experience with percutaneous operati-
ons under CT control in two patients and they review briefly experience from the literature
concerned with this problem.
