Summary: Stenosis of the larynx is the result of laryngeal injury or more frequently the result
of iatrogenic trauma after treatment of benign and malignant tumours. Patients with laryngeal
stenosis suffer from many impairments of the laryngeal function. The main signs are dysphonic and
dyspnoic disorders that sometimes require tracheostomy.
At the Clinic of Otolaryngology and Head and Neck Surgery we used the Montgomery laryngeal keel
for treatment of the anterior web of the larynx from 1994 to 1997. Since 1998 we used our specific
modification of the laryngeal keel.
The surgical technique is similar to the technique originally described by Montgomery.
Since 1994 we performed this operation in 6 patient with laryngeal stenosis. We used Montgomery´s
laryngeal keel in 3 patients and our specific modification of the laryngeal keel in the same number
of patients.
The mobility of the vocal cord and the voice improved in all cases. Two patients with tracheostomy
were decannulated. Perichondritis was observed in one case. Analysis of the silicone of laryngeal
keels did not prove any damage of the materials. Microbiological analysis did not reveal any
pathogenic germs.
