Summary:

The authors demonstrate in the submitted tase-record possible treatment of diabetic defects with a neuropathic etiology in a medical in-patient department. They emphasize in particular possible conservative treatment (perfect compensation of diabetes by means of an insulin pump, intense local treatment, antibiotic treatment, hyperbaric oxygen therapy), combined with minor surgical operations (incision, necrectomy, minor amputation, auto-transplantation of the slon). Treatment of a patient with the diabetic foot syndrome with a neuropathic etiology calls above all for a satisfactory compensation of diabetes, relief of local pressure, and in tase of infections, long-term antibiotic treatment. Team therapy is implemented best at a diabetological department with the opportunity of regular surgical consultations. There is evidence tkat this comprehensive therapeutic strategy which includes also education of the patient significantly reduces the number of major amputations and plays a part in the prevention of relapses of the diabetic foot syndrome.

Key words: diabetic neuropatky - diabetic foot - CSII - hyperbaric oxygen therapy - diabetes mellitus type 2.
