An Unusually Large Gastrinoma of the Pancreas

Summary:

       The authors present the case of a 44-year-old man with relapsing reflux oesophagitis. Two years after the onset of complaints the condition was complicated by abdominal pain and diarrhoea. Gastroscopic examination with the finding of multiple peptic lesions in the duodenal area, hypersecretion of gastric juices and a hyperplastic gastric mucosa in conjunction with the subjective complaints aroused suspicion of a gastrinoma. Computed tomography and endoscopic sonography revealed a focus in the head of the pancreas. The serum gastrin level was greatly elevated. Surgical revision confirmed a focus between the head and body of the pancreas, 4 × 4 × 3 cm in size. This well defined tumour located beneath the capsule of the pancreas was extirpated without the necessity to resect the pancreas. Histological examination confirmed a uniform tumour with a slightly eosinophil cytoplasm, with a subtle capsule. The finding was evaluated as an isolated solitary gastrinoma of the pancreas.
