The Application of Intracranial Stents in Endovascular Therapy for Brain Aneurysms

Summary:

       Purpose: The authors have presented results of endovascular therapy for brain aneurysms using the stent implantation and coiling. Material and Methods: 298 patients were treated within January 2000 – April 2005 for brain aneurysms in our department. In 36 of them (12 %) an intracranial stent was placed during an endovascular procedure. Indications for the stent placement were dissecting and fusiform aneurysms and those with broad necks. One stent was implanted because of periprocedural complication. Five patients have got two stents. A stentgraft was used in one case. All procedures were performed under general anaesthesia. An angio suite equipped with GE Medical System DSA machine was applied. Results: The retrospectively evaluated group includes 36 patients in whom 41 interventions were performed within 1/2000–4/2005 during which 38 stents were implanted 38 procedures were technically successful. Three procedures were technically unsuccessful. Two unsuccessful procedures were converted to surgical treatment, and balloon remodelling technique was used in one case. There occurred five periprocedural complications. The follow-up has shown satisfactory mid-term results in 25 patients, 4 patients had the aneurysm sac overgrowth, 2 patients died. Conclusion: Our results have shown widening possibilities of endovascular treatment especially in aneurysms previously not suitable for endovascular therapy. Improving technical equipment and experience may extend the reconstructive treatment possibilities.
