Summary: The authors submit a review of vascular and percutaneous intervention methods which can be used in the treatment of hepatic tumours. Since 1993 and 1996 resp, they treated thus 298 patients. During chemoembolization the main objective of treatment is in addition to aimed administration of a cytostatic complete ischaemization of the focus. In the superselective modification of the procedure they embolize the presinusoidal artery and via the sinusoidy also the branch of the portal vein. The other possibility of locoregional chemotherapy is administration of large doses of cytostatics most frequently via the hepatic artery. The cytostatic must rinse all liver segmenty. The preoperative embolization of the portal vein is a possible preparation of patients before resections of the liver. Indications of percutaneous thermoablation technique are limited by the cost of the procedure and the number, size and site of the foci. Percutaneous alcoholization is particularly useful in the treatment of hepatocellular carcinoma.
Percutaneous and angiographic procedures can be combined similarly as with surgery. The indication must be submitted for consideration to an interdisciplinary commission. The success of treatment is not evaluated according to the quality of the technical implementation of the procedure but by 3 to 5-year survival of patients. Correct technical implementation and suitable indication are prerequisites of success.
