Idiopathic thrombocytopenic purpura in pregnancy refractory to immunosuppressive treatment – case report

Summary:

       Idiopathic thrombocytopenic purpura (ITP) represents an autoimunne disorder complicated by isolated, variably severe thrombocytopenia. The incidence of this condition in pregnancy is 1–2 cases / 1000 uncomplicated deliveries. The main therapeutic approaches in pregnancy include corticosteroids, intravenous imunoglobulins (IVIG) and splenectomy. We present a case of ITP in 27 year-old female during her second pregnancy. The illness was resistant to therapy with corticosteroids (prednisolone 1 mg/kg) as well as to the combination treatment with cyclosporine A. High doses of IVIG led to a few days response only. Due to this poor response we decided to perform splenectomy at the beginning of the 3rd trimester. The surgery and subsequently childbirth of a healthy newborn passed uneventfully.
