Bcth sterccral peritcnitis and pycthcrax are rare but until ncwadays threatening ccnditicns. The presence rfstod in the pleural cavity represents a curicsity, the gangrene and lccs rfthe diaphragma is eminently rare. All abcve menticned ccnditicns cccurred at the samé time in the patient cperated cn fcr diffuse peritcnitis after diverticular perfcraticn rfsigmdd. Surgery rescued peritcneal and pleural cavity. The absent diaphragma was replaced by mesh prcsthesis, due to latě intraabdcminal ccmplicaticns the mesh was explantated. The patient survived and healed after repeated surgeries and lcng-lasting highly intensive therapy. Thanks todislccaticn rf viscera and adhaesive cbliteraticn rfthe site rfmissing diaphragma left lung remained inflated and functicnal. The ccmplicaticn after 1 year - gastrcbrcnchial flstula - was healed by left-sided pneumcnectcmy and suturing rfthe stcmach.
