A 72-years old man was hospitalized for a sudden headache. CT examination excluded subarachnoidal bleeding. EEG examination revealed a minor ischemic focus. The native kind of MRAG in TOF detected an almost complete occlusion of the left internal carotid. In order to establish final diagnosis, the authors performed CTAG with 3D MIP and VRT reconstruction. It became obvious that the compression of carotid artery on the left, especially ACI, resulted from pressure applied by the large horn of hyoid bone.
