Severe Organophosphate Poisoning in Toddler

Summary:

       Intoxication of organophosphate insecticides is rare but life threating condition in young children. Muscarinic, nicotinic and CNS symptoms are manifestated in clinical presentation of acute poisoning due to irreversibile inhibition of acetylcholinesterase which leads to accumulation of acetylcholin in nervous system. We describe a 33-month-old boy with severe CNS depression and hypotonia after organophosphate poisoning which was confirmed by toxicologic investigation of gastric contens. On the basis of the result of a toxicologic investigation treatment with atropine and obidoxime was started to the clinical features of atropinization. Treatment with obidoxime was finished after four days. The boy was extubated on the 8th day of ventilatory support. Next day continuous treatment with atropine was stopped. Presented case-report of organophosphate intoxication in patient with typical clinical and biochemical findings document severity of this poisoning in young children.
