Summary:
The aim of the work was to inform about advantages and disadvantages of surgical treatment
of lower concha by means of microdebrider.
The injection infiltration of lower concha by an anesthetic with adrenalin the microdebrider
knife is inserted from the injection site at the anterior margin of lower concha under the mucous
membrane. By pressing the knife, which acts as a raspatorium, a submucous tunnel is formed at
the medial side of the bone basis of the concha. Te submucous tissues are reduced by means of
a 2.9 mm microdebrider produced by Medtronic Xomed in the oscillation mode at 3.000 revolutions
under the control of the endoscope until an adequate size of the concha is reached. The operation
is finished by tamponade of nasal cavity.
The shaver partial turbinectomy was performed in 50 patients. The subjective evaluation of nasal
patency was assessed as excellent in all patients within a time lapse of at least six months.
Crustae or atrophic rhinitis were not observed, synechia between the septum and lower concha
was present in one case.
The submucous shaver partial turbinectomy is a safe method serving for reducing the volume of
lower concha with an acceptable morbidity in patients suffering from nasal obstruction.
Preservation of mucosa results in a rapid healing and decreased risk of crust formation or denudation
of the bone. It fulfills the conditions of functional approach to treatment of lower concha
diseases.
