Summary:
The cross-sectional non-randomized questionnaire survey was part of thePHARE
project of the Ministry of Health "Finance and Primary Health Care" comprising
one sub-project "Health Promotion". The "KAP test" study included questionnaires
on knowledge, approach and practice in the sphere of health promotion in health
professionals engaged in primary care. The questionnaires were completed using
personal interviews, a small proportion was dispatched by post. The survey
comprised 658 complete questionnaires from 312 physicians and 346 nurses who
worked in out-patient departments of general practtioners and paediatricians.
The KAP test comprised 16 questions (incl. 7 "K"- knowledge, 6 "A" attitude and 3
"P" practical). The results and statistical evaluation were processed in the SPSS
programme.
The questions concerning knowledge such as health dimension, calculation of the
BMIindex , differencebetweenhealth promotionandhealtheducation,preventive
provisions as regards hypertension, plan of preparation of health promotion
projects, vaccination and screening within the framework of prevention revealed
better knowledge of doctors as compared with nurses. Gaps were found e.g. as
regards calculation of BMI, timing of screening, explanation of differences between
health education and health promotion. From questions "A" on the attitude
to health promotion ensues the view that for human health the physician as well
as the patient are responsible and that health promotion and therapy are of equal
value. As regards factors of social development the greatmajority of respondents
ascribes an important part to the development of health services and reduction
of poverty,whereby politicaland financial problemsdo not play according to them
an important role. According to the view of respondents health education should
comprisehealth promotionandmassmedia,counselling of patients,immunization
and even elevation of taxes on tobacco products. As regards practical questions
"P" on prevention of breast cancer doctors prefer an individual approach more
than nurses. As to practical questions the most interesting one was why not more
is done in the sphere of health promotion. The greatest obstacle for health
promotion activities is lack of time. Doctors and nurses mention as reasons also
inadequate education in this sphere. Obviously practical education and training
in individual counseling are lacking. Next there are significant financial reasons,
description of work and the reply "I do not know" which could signalize the desire
for certain organizational changes prefering preventive measures within the
framework of the financial policy of the health insurance company.
Based on the survey and financial support of PHARE an intense training model
was selected and a seminar for 20 specialists in health promotion and PZS.
