Stents in the endovascular treatment of wide-neck intracranial aneurysms: Initial clinical experience

Summary:

       A retrospective analysis of endovascular therapy using stents in 8 patients with 9 wide-neck intracranial aneurysms is reported. One dissecting aneurysm of the basilar artery decreased in size and was not indicated for further invasive treatment and there was one technical failure in the nonruptured middle cerebral artery aneurysm in the patient with two wide necked aneurysms. The therapy with using dedicated bare stents and one stentgraft in all seven remaining aneurysms was successful. In 3 out of these aneurysms the previous endovascular approach failed. In spite of more complex navigation and higher risk of complications the dedicated intracranial stents have brought possibility of improved effectiveness of wideneck aneurysm endovascular treatment.This isdue to stabilization of coils position and luminal remodelling of the parent artery which is both mechanical and hemodynamical.
