The Different Influence of Helicobacter pylori Eradication on Dyspeptic Symptoms in Peptic Ulcer and Functional Dyspepsia. Long-term Follow-up

Summary:

       The goal of our study is to compare long-term influence (between 2 to 12 years) of Helicobacter pylori (HP) eradication on main symptoms (pain and epigastric discomfort and vomiting) of the patients suffering from peptic ulcer disease (PUD) and functional dyspepsia (FD). Helicobacter pylori infection was proved in all patients during the first examination by rapid urease test and/or histology. Total number of investigated and fol-lowed-up patients were 114 suffering from fresh duodenal ulcer (87 successfully HP eradicated, 27 pts without eradication) and 150 patients suffering from functional dyspepsia (64 pts successfully HP eradicated, 86 pts without eradication).The symptoms of eradicated peptic ulcer patients were significantly improved. The diffe-rence between diminishing symptoms between eradicated and non-eradicated patients was statistically signi-ficant for each followed symptom (epigastric pain and discomfort p 0.001, vomiting p 0.01). Even after HP eradication and without present ulcer dyspeptic symptoms outlast in some ulcer patients. We can explain it by coincidental tendency of these patients to functional dyspepsia. The prevalence of dyspeptic symptoms after HP eradication in long term follow-up in FD patients moderately decreased. In contrast with peptic ulcer patiens similar changes were seen also in FD patiens without HP eradications. The difference in decreasing symptoms between eradicated and non-eradicated patients was statistically insignificant in FD except vomiting where in a non-eradicated group was significant decrease than in eradicated group (p 0.05).Our results support an opinion that HP infection has no influence on dyspeptic symptoms in FD.
