Summary: The use of non-invasive methods for diagnosis of vascular disease has increased rapidly but the
complicated anatomy of aortic arch and its branches is sometimes difficult to interpret when
presented as sectional images. The development of three-dimensional reconstruction techniques
promises to overcome some of these problems. The authors describe two patients ill with aneurysms
of aberrant right subclavian arteries. One of them had thrombosis of the aneurysm. Non-invasive
spiral CT imaging with 3D image reconstruction clarified the anatomy preoperative and avoided
the need for postoperative angiography. Clinical management and preoperative planning may be
based succesfully on non-invasive CT imaging and 3D reconstruction.
