Reference intervals, optimal and target values in the context of the clinical interpretation of lipid profile

Summary:

       The authors offer the solution of the problem with lipid reference intervals used in laboratory information systems (LIS). Definitions of reference interval (2.5–97.5 percentile of concentrations measured in a defined population or measured in reference individuals with known health status), optimal values (best values for the discrimination of cardiovascular risk) and target values (therapeutic target adjusted to cardiovascular risk in specific groups of patients) are given. The authors reject the use of reference intervals as defined above in LIS and recommend the use of optimal values harmonized with Czech national societies guidelines for upper limit of cholesterol (Ch), LDL cholesterol (LDL Ch) and triglycerides (TAG), or lower limit of HDL cholesterol (HDL Ch), respectively. They recommend to use the NORIP study limits for lower limits of Ch, LDL Ch and TAG, or upper limit of HDL Ch, respectively. Similarly, the same approach is recommended for children. Target values are treated in a form of simple and instructive algorithm, based on national recommendation for the prevention of cardiovascular diseases in adults.
