Summary:
The authors describe their own experience with a transcondylar approach to the ventral brain stem,
lower clivus and foramen magnum. Experience is presented not only on cadaver models, but also on
clinical cases. The transposition of the vertebral artery, C1 hemilaminectomy and resection of the
occipital condyle with a part of the lateral C1 mass are crucial for the approach. The approach makes
a safe and radical solution of ventral corresponding pathologies of this region with minimal retraction
and excellent visualization of the neurovascular structures of craniovertebral junction possible.
Indications, advantages, disadvantages and potential risks are discussed.
