Mechanical percutaneous extraction of embolus in acute stroke: A case report and
review of the literature

Summary:
A case report of a man with an acute middle cerebral artery embolism and with NIHSS score of 15 is presented.
Urgent angiogram revealed „T“ type occlusion of the left internal carotid artery. Using intra-arterial
rTPA infusion, the anterior cerebral artery was recanalized. Successful mechanical removal of embolus
in the M1 segment of the middle cerebral artery was performed after continuous rTPA infusion failure. A 4-
mm micro-loop snare for foreign body extraction was used. Techniques of endovascular recanalization in
patients with acute cerebral ischemia are discussed, including local thrombolysis, aspiration thrombectomy,
balloon angioplasty, stent implantation and mechanical extraction of thrombus.
