Summary:
The company Roche Diagnostics GmbH organized an international study „International Elecsys 1010 Study –
Cardiac Marker“ from September 1998 till January 1999. 47 laboratories from 13 European countries took part
in this project.
Our report includes preliminary study results obtained in the biochemical lab of the 2 nd Internal Department
of the University Hospital in Hradec Králové.
The following cardiac markers were estimated: troponin T was taken using the 3 rd Generation Elecsys Troponin
T STAT Immunoassay kit, myoglobin using the Elecsys Myoglobin STAT Immunoassay kit and CK-MB (mass)
using Elecsys CK-MB STAT Immunoassay kit. Concentration of all three parameters was measured by electro -
chemiluminiscence on the Elecsys 1010 apparatus. The apparatus and kits for sampling, calibration and control
were made by Roche Co. The sera of 46 blood donors, 32 women and 14 men, were tested. The troponin T (cTnT)
values were lower than 0.01 mg/l in all persons irrespective of gender. The myoglobin values were 37.12 ± 7.93 mg/l
in women and 39.81 ± 6.49 mg/l in men. The following values were determined for CK-MB (mass): 0.956 ± 0.66 mg/l
in women and 1.191 ± 0.64 mg/l in men. Furthermore the course of changes of myocardial markers in five patients
with acute myocardial infarction (AMI) was monitored. Three patients were diagnosed with Q wave AMI and 2
patients with non-Q-wave AMI.
Two patients underwent percutaneous transluminal coronary angioplasty, one was treated by thrombolysis
with streptokinase and two were heparinized. Early cTnT and CK-MB (mass) peaks in three patients proved
successful recanalization.
Key words:
troponin T, creatinine kinase-MB, myoglobin, acute myocardial infarction, diagnostic strategy.
