III. chirurgická klinika l. LF UK, Praha

Summary:

The authors try to explain possible procedures of the diagnosis and therapy of patients with Barrett’s oesophagus (BO). In the course of 10 years they followed up 57 patients with a histologically confirmed BO. The mean follow-up period was 33.9 months. From the total number of patients 25 were indicated for an anti-reflux surgery. A postoperative gastrofibroscopic (GFS) check-up was made in 22 patients. In 15 patients macroscopic pathological changes were present no longer in the distal oesophagus. Sixteen patients were indicated for resection, 15 primary resections and one after previous intrathoracic fundoplication on account of persisting dysplasia, and five on account of nondilatable reflux stenosis. A total of 17 patients were indicated for surgery – 11 were contraindicated on account of internal polymorbidity, 5 patients refused surgery or did not attend a further check-up, and one patient with advanced carcinoma had palliative treatment with a stent.
