Summary: The author discusses the problem of endoscopic treatment of acute pancreatitis. In his opinion it is indicated only in the biliary form of acute pancreatitis. He briefly draws attention to the three stages of endoscopic treatment: diagnostic ERCP, papillosphincterotomy and removal of concre- ments from the biliary pathways. Endoscopic treatment is indicated unequivocally in the severe form of acute biliary pancreatitis and in patients with jaundice and cholangitis. Mild forms of acute biliary pancreatitis do not call as a rule for urgent ERCP. It should be however used if the patient's condition does not improve after classical conservative treatment. Endoscopic treatment is nowadays considered not only a therapeutic method but is important also in the prevention of possible further attacks of acute pancreatitis
