Summary:

The main treatment of idiopathic thrombocytopenic purpura (ITP) in adults is based on long-term application of corticosteroids. The different treatment modalities including surgery may appear necessary in patients with corticoresistant ITP. Demonstrating the case of a 36 year-old man suffering from corticoresistant ITP and severe thrombocytopenia the authors discuss contemporary treatment possibilities and call attention to the preparation for splenectomy by means of activated factor VII.They used NovoSeven (Novo Nordisk) after the preparation by concentrated thrombocytes had failed. Splenectomy went problem free, the patient healted primarily and the amounts of thrombocytes became stabilised.

Key words: idiopathic thrombocytopenic purpura (ITP) - corticoresistency - splenectomy thrombocytopenia - activated factor VII - Novoseven.
