Migrainous Stroke

Summary:

       Migraine is a frequent disease even in children, migrainous stroke as a complication of migraine is however very rare. The authors describe the case-history of a 12-year-old girl who after a three-month history of migraine without aura developed during an attack of headache dextrolateral hemiconvulsi- ons with post-attack hemiplegia with a CT correlate of malacia in the left putamen. The motor disorder receded within 24 hours. In the EEG there were atypical sharp waves in an occipital position on the left, the angiographic finding was normal. A similar condition with a milder course occurred again after 18 days, and 8 months later again headache, hemiconvulsions on the left side, reversible hemiparesis without a CT and angiographic correlate. The patient was treated at first with anti-epileptics and flunarizine, after normalization of the EEG after 15 months treatment was terminated. During the subsequent six years with lisuride and flunarizine treatment there was only sporadic headache without aura and a vegetative symptomatology. During the last three years there were no complaints, no treatment. Migrainous stroke is a rare cause of focal cerebral ischaemia in children.
