Summary:
Aim: To reffer about first experience with the use of selfexpanding
stents (SES) in cases with colon obstruction.
Material and methods: Comparative choice analysis of
the group of five patients undergoing emergency operation
and five patients operated on after SES placement
and preoperative preparation.
Results: Age of patients and lenght of hospital stay were
comparable. Morbidity rate was 40 % in the group with
SES comparing 80 % in the emergency group. Mortality
rate was 0 in the first group, 20 % in the second. The
count of P-POSSUM in the group with SES has shown
significant risk decrease after the preoperative preparation.
Average costs of the treatment in the SES group
were 149 651 Kč, comparing 304 612 Kč in the emergency
group. The most expensive treatment in this group
cost more than 500 000 Kč.
Conclusion: Solution of colonic obstruction by means of
SES seems to be advantageous method ot treatment comparing
usual therapuetic modality. Assesment of PPOSSUM,
morbidity and mortality rate, and at the end,
the cost effectivness, as well, speaks for introducing SES
as the bridge to elective operation. It is a good mathematical
representation of the benefit of this therapeutical
modality, as well.
Key words:
emergent stent, obstructed ileus, colon cancer,
POSSUM, cost effect.
