Summary: 5–20% paragangliomas originates from extraadrenal tissue paralelly to occurrence of anatomic components of
vegetative nervous system. This distribution corresponds to embryological development of paraganglias from
cells of neural crest. This tumors can extraordinary rise also except this usual area. Case report of patient
whose paraganglioma was detected in the mesenterium but suggested to be a lymphoma due to enlarged
lymfatic nodules and Epstein-Barr virus and cytomegalovirus reactivation has been presented. This extremely
unusual paragangliom locality was described only in four cases till now. A normal neurovegetative tissue of
paraganglioma occurs only around mesenteric artery branching. Hypothetically speaking the explanation for
paraganglioma in mesenterium seems to be a consequence of the migration of neural crest cells ventrally
around this vessels. Case report describes differential diagnostic consideration and also unusual pseudotumorous
reaction to foreing body material by desmoid proliferation which complicated surgery. The aim of the
paper is to stress the possibility of unusual localisation of paragalioma and differential diagnostic consideration
during the course of described ilness.
